Surrey GPs Predict Cost-Effective Commissioning

NHS Surrey Commissions Risk Stratification Tool from Docobo Ltd to Reduce Unplanned Admissions

02 FEBRUARY 2012, SURREY, UK
Summary
Innovative technology developed by UK company Docobo Ltd, will support NHS Surrey in their initiatives to manage patients at home and reduce hospital admissions. NHS Surrey has commissioned Docobo Ltd to supply its new risk stratification tool, ArtemusICS™, that will enable GP practices in Surrey to better target their resources to deliver better outcomes for the 1.2 million people living in Surrey.
The tool takes patient data from both Primary and Secondary care and, through detailed analysis, will identify patients at risk of requiring future healthcare resources and who would likely benefit from a specific Long Term Condition intervention.   The data is presented in a convenient ‘dashboard’ format so that information can be easily accessed via secure web access by authorised staff at GP practices.    Data can be examined based on groups of patients, such as by practice or locality, or by clinical condition.  Each patient is allocated a ‘risk score’ which predicts their risk of a hospital admission in the next year.  Their GP can view this data and use it to direct their resources more effectively. This enables them toprovide more targeted personalised care and will help to identify and treat health problems earlier. Furthermore, the patients’ risk scores can be monitored regularly to examine the effect effectiveness of treatments and interventions.
Quotes
"We chose the ArtemusICS tool because it enables clinicians to target patients who would benefit from greater intervention, helping to reduce unplanned hospital admissions, which can be very distressing. Using this tool helps clinicians achieve the best possible outcomes for their patients and it also enables them to allocate their resources to best effect. The tool creates dashboards, which are easy to access, and give clinicians valuable insight into primary and seconday care data, helping them identify those most at risk and enabling them to tailor care to meet their needs. Docobo have worked with us to develop the tool to meet our requirements , and have delivered a quality product that is already making a real difference to GPs. " Christine Ratcliffe, Associate Director of IT Programmes, NHS Surrey
"‘We understand that many of our patients prefer to be treated in their homes rather than in hospital. ArtemusICS will help us identify patients who are at risk of a hospital admission and enable us to provide those patients with more targeted, personalised care to keep them in their homes where appropriate and possible. It will give us a clearer understanding of the healthcare needs or our patient population and help us to commission services appropriately" Dr. Peter Stott, GP, Tadworth Medical Centre & GP Lead Medlinc CCG
"Docobo is proud that our ArtemusICS system has been selected by NHS Surrey. We are particularly delighted to have had the opportunity to work closely with Surrey to develop this tool to precisely fit their needs and we look forward to working with the GP practices in Surrey to roll this out." Adrian Flowerday, Managing Director, Docobo Ltd
